First of all: if you're on an older iPod Touch, you may not have an external speaker. If that's the case, this app won't be much fun for you. You'll be able to hear the sounds through the headset, but that's about all you can do on an older iPod.
Make sure your headphones aren't plugged in, and that you're not paired to a bluetooth headset. The sounds will play though whatever the "current" speaker is. If headphones are plugged in, they are current. If bluetooth is paired, that may be current.
Here's the rule of thumb: whatever speaker music would come out of right now in iPod mode, that's the speaker we're going to play sounds through.
The program attempts to override the ringer/silent switch, and should tell you if your volume is down. So, if it looks like the app is working (the red button depresses when you push it, and pops back up after a couple seconds), check to see where your sound might be routed to.
If all else fails, reboot the phone, my friend.
This was a frequent problem with iPhone OS 2.0 when you had many downloaded apps on your phone. If you haven't upgraded your firmware to at least version 3.1 yet, you definitely should.
Try rebooting your phone. This problem can happen if memory has gotten fragmented over time (it happens, nothing to be alarmed about!) and a reboot will fix it. If you still have this problem after a reboot, try deleting the app from your phone and re-install it.
Note that no one has reported this actual problem yet, it just seems like it's a common problem with iPhone apps, and that's the common remedy. Only Apple knows why it works, and it's just here to cover our bases.
